Ultherapy vs Thermage vs Sofwave vs HIFU vs Thread Lift in Seoul: Which One, How Much, and What to Expect

The short answer

They are not interchangeable, and the right one depends on what is sagging and where the laxity sits, not on which device is newest.

How does each one actually work?

Ultherapy delivers micro-focused ultrasound to precise depths, including the SMAS layer at 4.5 mm that surgeons tighten in a facelift. Each "shot" creates a line of tiny thermal coagulation points; collagen remodels around them over 2–3 months. It is the only non-surgical device with imaging, so the operator sees the layer being treated. The original rater-blinded study (Alam et al., J Am Acad Dermatol, 2010) showed brow lift in the majority of patients at 90 days; later Korean cohorts reported similar results in Asian skin with adjusted energy settings.

Thermage FLX heats a broad volume of dermis and upper fat with monopolar radiofrequency while cooling the surface. It does not reach the SMAS. Its strength is skin quality: fine crepe, pores, mild laxity, and the eyelids (Eye Thermage uses a small tip). Periorbital tightening was shown in the first multicenter study (Fitzpatrick et al., Lasers Surg Med, 2003), and the FLX generation reduced pain with vibration and a larger tip.

Sofwave uses synchronous ultrasound beams to heat a band at 1.5 mm, mid-dermis. It deliberately stays shallow, which is why it is nearly painless and safe over thin areas, and why it does not lift heavy tissue. FDA-cleared for lifting of the brow, submental area and neck. It is a texture-and-fine-line device that is often sold in Korea as a lifting device; expect skin improvement, not a jawline change.

Korean HIFU (Shurink Universe, Linearz, Ultraformer and others) is the same physics as Ultherapy at lower energy per shot and lower cost per shot, without imaging. Results are real but shorter-lived; Korean patients typically repeat every 6–12 months. It is a sensible first device for mild laxity and for people who want to test their response before paying for Ultherapy.

Thread lifts place barbed absorbable threads (PDO, PCL or PLLA) under the skin to physically reposition tissue. The lift is visible on the day. The thread dissolves over 6–18 months and leaves some collagen behind. A Korean outcomes series (Suh et al., Dermatol Surg, 2015) reported good satisfaction with a bruising-and-swelling period of about two weeks and occasional dimpling or asymmetry that resolved.

Which one for which problem?

Your concern Usual first choice Why not the others
Jowls, blurred jawline, double chin starting Ultherapy (or HIFU if budget) Thermage and Sofwave do not reach the layer that is sagging
Skin looks thin, crepey, tired; mild laxity Thermage FLX or Sofwave Ultherapy is overkill and more painful for a surface problem
Upper and lower eyelid skin laxity Eye Thermage Ultrasound devices are limited near the globe
Cheeks dropped, want visible lift now Thread lift, often combined with Ultherapy or HIFU later Energy devices take 2–3 months and lift less
Pain-averse, thin skin, first time Sofwave or Shurink Ultherapy without sedation is a hard first experience
Significant laxity in the 50s and beyond Honest answer: these devices will disappoint; discuss surgery Non-surgical devices cannot lift what surgery lifts

"Ulthermage": why Korean clinics combine them

You will see packages called 울써마지 (Ultherapy + Thermage) everywhere in Gangnam. The logic is layered: Ultherapy for the SMAS, Thermage for the dermis. The literature on same-day combination is thin, and the main practical question is pain and cost. A common Korean approach is to stage them a few weeks apart. If a package is offered, ask which device is being done first and at what shot count for each; a bundle price is only comparable when the shot count for each device is written down.

How many shots do I actually need?

Shot counts are where quotes become incomparable.

Ask for the shot count on the receipt, not just the total.

How do I know the Ultherapy or Thermage tip is genuine?

Both devices use single-use cartridges or tips with a shot counter. Reputable Korean clinics open the tip in front of you and show the counter before and after. Korean patients ask for this routinely (the phrase is 정품 개봉 보여주세요, "please open the genuine product in front of me"), and some clinics advertise a refund guarantee if the tip is not genuine. If a clinic will not show the tip, that is a reason to leave, not to negotiate.

Can Ultherapy, Thermage or HIFU make my face look thinner or more sunken?

This is the question that worries thin patients most, and it is legitimate. Case reports in the literature describe facial fat loss after focused ultrasound, and practitioners generally attribute it to too much energy at the 4.5 mm depth in a face without enough fat to spare. The practical implications:

If a clinic says the risk does not exist, it is wrong. If it says the risk is managed by assessment and settings, that is the correct answer.

Does it hurt, and should I take sedation?

Ultherapy at full energy is the most painful of the group; Korean clinics commonly offer sleep sedation (수면마취) for 300 shots and up, sometimes included in the price, sometimes ₩50,000–150,000 extra. Thermage FLX, Sofwave and most HIFU are done with numbing cream and are tolerable for most people. Thread lifts use local anesthetic injections; the insertion is tolerable, the next week is the uncomfortable part.

If you are traveling alone, note that sedation means you cannot drive and should not fly the same day.

How do Seoul prices compare?

Treatment (same unit) Seoul first-visit Tokyo United States Taipei
Ultherapy ~300–360 shots ₩600,000–1,089,000 ¥242,000 (360 shots) US$2,300–5,500 (full face/neck) NT$54,000–90,000 (full face)
Thermage FLX 600 shots ₩1,190,000–2,200,000 ¥188,000–363,000 US$1,800–2,500 (lower face) NT$60,000–80,000
Thread lift per thread ₩11,000–219,000 ¥30,000–60,000 US$200–500 NT$5,000–25,000

Lifting devices are where the Seoul discount is largest, typically 2–4× cheaper than Japan and the US at the same shot count. That is also why they are the most heavily upsold treatments for visitors; see the quote-vs-bill page before you book.

Frequently asked

Ultherapy or Thermage, which is better? Different layers. Ultherapy for sagging you can lift with your fingers along the jaw; Thermage for skin that looks loose but does not drop. Many Korean patients do both, staged.

Is Sofwave the same as Ultherapy? No. Same energy type, one-third the depth. It is a skin-quality treatment that happens to be marketed as lifting.

Is Korean HIFU (Shurink) a cheap Ultherapy? Roughly, yes: same principle, less energy, no imaging, shorter duration, much cheaper. A reasonable choice for mild laxity or a first try.

How long after Ultherapy can I fly? Same day is fine without sedation. With sedation, next day.

Can I do a thread lift and Ultherapy on the same trip? Clinics do combine them. The order is Ultherapy first, threads after. Applying focused ultrasound over freshly placed threads can damage them, so the reverse order is avoided.

My skin feels looser two weeks after Ultherapy. Is that normal? It depends on the case. Sometimes it is swelling going down, which can feel like softening before collagen remodeling at 2–3 months and recovers on its own. Sometimes it is cheek fat that has reduced, which is real sagging and is the fat-loss scenario above. The treating doctor should assess which one it is.
